
Handicap placards can be obtained through our office. You must present a “Parking Placard Application for Persons with
Disabilities,” filled out by your physician, physiotherapist, or occupational therapist. They will fill out the “Eligibility” section, checking off one of the short term, long term, or permanent disability boxes. These forms can be picked up from our office or can be found at your doctor’s office as well. You will either need to bring in the placard application yourself (with valid ID), or have your legal guardian or person with Power of Attorney sign it for you, and bring it in.
Alternately, you can fill out an authorization form, which the authorized person must bring in, along with your placard application, and his or her identification.
Depending on your medical condition, you may be required to obtain a drivers road test to determine your eligibility to drive.
The current fee for a handicap placard is $9.45.
